Who can have a Home Medicines Review?
Your GP decides if a Home Medicines Review is clinically appropriate. In general, it’s available for people living at home who have a Medicare or DVA card and may be at risk of problems with their medicines.
Basic eligibility
- You have a current Medicare or DVA card.
- You live in your own home or a community setting (not as a permanent resident in residential aged care).
- Your GP believes you are at risk of, or already experiencing, medication-related problems.
- There is an identifiable clinical need for a Home Medicines Review.
Common reasons your GP may suggest an HMR
- You take several regular medicines, especially five or more.
- You see more than one doctor or get medicines from different places.
- You’ve recently started new medicines or had your doses changed.
- You’ve recently been in hospital or to the emergency department.
- You feel unwell after taking medicines or are worried about side effects.
- You sometimes miss doses or find your schedule hard to manage.
Who should you talk to?
- Start with your GP or practice nurse – they decide if an HMR is right for you.
- You can bring a family member or carer to your GP appointment and the home visit.
- If you look after someone else, ask their GP whether an HMR could help them.
Only a GP can write a referral for a Home Medicines Review (Medicare item 900). They’ll explain how the service works and any costs related to the GP consultation.

What is a Home Medicines Review (HMR)?
A Home Medicines Review is a structured, GP-initiated service where an accredited pharmacist reviews a patient’s medicines at home, then provides a written report and recommendations back to the GP.
Is there a cost to the patient?
The pharmacist’s review itself is generally funded for eligible patients under existing government programs. GP consultation fees are determined by each practice. Patients should confirm any potential costs with their GP clinic.
